When choosing from the additional choices if the selection does not already exist as an option choice it will be added at that time with the default settings.
The default setting for “Add Extended Text” is ‘No’. Once the option choice is added it would need to be manually updated if it requires different settings. You could explore an extension to change the behavior to set it to a different value when the option choice is created through the additional choices.
Related to this, it is in our road map to introduce ‘option choice templates’ where the idea is you can specify a template with the default or standard settings you want to apply when creating option choices (such as when they are created through the additional choices).
There is no confirmed timeline on when this will be added to the product however it is in our plans.
